Takayasu's disease revealed by a massive aortic regurgitation in later life

Abstract

The aortic insufficiency secondary to Takayasu's disease is relatively rare and is a major mortality
factors in this nonspecific inflammatory disease.The diagnosticrepose on Ishikawa criteria in 1988,
but remains too restrictive excluding patients older than 40 years at the beginning of the
disease.The youngest age of revelation described in the literature is 6 months, while the most
reported age is 75 years. Hence the peculiarity of our reported cases where the diagnosis is made at
the age of 73, revealed by severe aortic regurgitation. Takayasu's disease in this case is selected for
association with a diffuse inflammatory disease of the supra aortic trunks, stenosis of the
subclavian artery and positivity of inflammatory balance.Disease management is primarily based
on immunosuppressants, whose effectiveness depends on the evolutionary stage of the disease. The
operating management for its part takes some difficulties including the problem of the operating
timing, the choice of the surgical technique, and especially postoperative anastomotic
complications. Hence the need for corticosteroid therapy and surgical pre postprocédure to avoid
these complications.
