Hyponatremia and Psychotropic drugs : A case report

Abstract

Epidemiological data highlight the relatively high frequency of hyponatremia in the psychiatric
population secondary to the use of psychotropic drugs including antidepressants. This is a
potentially dangerous comorbidity. It is secondary to inappropriate secretion of antidiuretic
hormone (SIADH). It appears usually during the first weeks of treatment. Its occurrence is bounded
by several risk factors to look for, in order to verify the serum sodium in the high-risk patients. The
clinical hyponatremia extends from asymptomatic forms to more pronounced and severe ones.
Normalization is often obtained at the end of treatment coupled with fluid restriction and normal
sodium diet. In severe cases, hypertonic saline perfusions are indicated. These Data incite the
clinical and laboratory monitoring. Initiation of antidepressant treatment in a patient at risk must
be preceded by a prior electrolytic analysis followed by biological monitoring in the early
treatment .
